Young Adult Transition Team
Making the transition to adulthood can be challenging, especially if you’re struggling with behavioral health concerns. Our Young Adult Transition Team (YATT) can help.
Care team members will help you gain life skills while also addressing your mental health needs. And we’ll make sure that your treatment plan supports your independent living goals.
Eligibility
YATT is for young people ages 16-21, who meet one of the following criteria:
- You are a KMHS client
- You are a resident of Evergreen Bright Start, a 30-unit apartment complex for vulnerable young adults. It is operated by Bremerton Housing Authority.
Young adults up to age 24 may participate in Supportive Housing Services.
How to Access Services
KMHS Clients
Ask for a referral during your KMHS intake or from your clinician.
Evergreen Bright Start Residents
If you are not a client, visit the KMHS office located in Evergreen Bright Start to get started.

- Our Employment Specialist can help you build job-seeking skills
- Our Housing Support Specialist can help you build skills to find and keep housing
