Support for Adults Living with Severe and Persistent Psychosis
Assertive Community Treatment (ACT) offers rehabilitation and support services to adults experiencing severe and persistent psychosis. We focus on the individual’s needs, goals and strengths, to pave their path to recovery and independence.
Contact us if you, or an adult in your life is experiencing persistent and severe psychosis and it is negatively affecting any area of life, including:
- Legal troubles related to symptoms/psychosis
- Increased self-isolation as a result of symptoms
- Frequent hospitalizations (2-3 times a year) due to psychosis symptoms
Your Recovery Team
Our team of experts is ready to provide individualized support in a variety of areas including:
- Career and learning goals
- Preventative healthcare
- Developing healthy lifestyle choices
- Medical services
- Medication treatment and education
We also have staff available to offer peer support and mentorship.
Eligibility
- Have a diagnosis of schizophrenia, or schizoaffective, schizophreniform or other persistent psychotic disorder
- Age 18+
- Kitsap County Resident
- Primary cause of psychotic symptoms not known to be from the temporary effects of intoxication, major depression or other medical conditions
- Experiencing significant impairment in one or more areas of life as a direct result of psychosis symptoms
