A Bridge to Independence

Making the transition to adulthood can be challenging, especially if you’re struggling with behavioral health concerns. Our Young Adult Transition Team (YATT) can help.

What We Do

Our team of mental health professionals works collaboratively with youth ages 16-21. We can help you gain independent living skills while also addressing your individual mental health needs, always ensuring that your treatment plan supports your independent living goals.

Life Skills Groups

You can choose to participate in one or more of these practical skills groups, which are taught in 12-week blocks:

  • Self-Care –  Topics include nutrition, exercise, sleep hygiene, wellness resources
  • Future Focus – Topics include housing, transportation, finances, continued education, job search skills
  • Social Forum – Topics include healthy relationships

Our Summer Group Therapy program focuses on community activities and participation.

Locations

The YATT program is available at our North Kitsap, Sheridan (Bremerton) and South Kitsap locations. Transportation assistance may be available. Ask your KMHS clinician for a referral.
